PACE Eligibility

Siouxland PACE Serves Individuals
Who Meet the Following Requirements:

  • 55 years of age or older.
  • Determined by IME Medical Services to need the level of care required for Iowa nursing facility services.
  • Reside in the service area of the PACE organization.
  • Meet any additional program-specific eligibility conditions imposed under the PACE program agreement.

Insurance Coverage & Program Fees:

If you are eligible for both Medicare and Medicaid or Medicaid only, you will make no monthly premium payment to Siouxland PACE. Plus, you will receive all PACE services including prescription drugs.

If you have Medicare and are not eligible for Medicaid, there is a monthly fee for the program and a Part D premium. In cases when a participant has neither Medicaid or Medicare, then the participant pays three payments to the program: Medicaid, Medi­care and Part D premiums. Our Enrollment Coordinator can assist you with additional information on program costs.
