Siouxland PACE (Program of All-Inclusive Care for the Elderly) is a non-profit organization that coordinates and provides all health care and supportive services to enable frail individuals to remain living in the community for as long as possible.

Answers to Frequently Asked Questions about Siouxland PACE Services:
Can I join without attending the Therapeutic and Recreational Center?
Yes. Your participation and attendance is based on a mutual agreement between you and your team of health care professionals. Your PACE team will work with you to determine the right combination of services to meet your unique needs.
Do I have to change doctors?
It is necessary to change to a physician who is familiar with PACE procedures and the PACE team philosophy. This involves changing to the designated Siouxland PACE physician in. We will help make the transition easy.
What happens if I need to see a specialist?
If you need to see a specialist, such as the cardiologist or the pulmonologist, we will make the arrangements and provide the transportation. A list of specialty providers is given to participants at the time of enrollment.
What happens if I am no longer able to live at home?
Siouxland PACE will do everything possible to keep you in your own home. However, if you are no longer able to remain safely in your home, Siouxland PACE will look for a facility such as assisted living or a nursing facility to meet your needs. Siouxland PACE continues to be responsible for the participant's care.
What happens if I join Siouxland PACE and I don't like it?
Siouxland PACE staff will treat you as a friend and support you in your care needs. However, if you choose to discontinue your services with Siouxland PACE, you may disenroll from the program. A Siouxland PACE Social Worker will assist you.
