St. Luke’s Corporate Health and Wellness Program

St. Luke's Health System in Sioux City, Iowa is the hospital of choice for bringing health and wellness to employees. St. Luke's Center for Occupational Health Excellence, in partnership with Four Seasons Health Club, has created a customizable Corporate Health and Wellness program that encompasses medical health services, wellness and fitness to meet the needs of each company and the individuals it employs.

Health Risk Appraisal
The St. Luke's "Life Steps" Health Risk Appraisal is an online, interactive questionnaire used to identify an employee's overall health risks, giving them a better understanding of their personal health data including:

• Laboratory results
• Lifestyle habits
• Current health status
• Chronic conditions
• Future disease risks

From an employer's perspective, "Life Steps" supplies multiple report options to provide aggregate data from employee questionnaire responses. This data, in turn can guide employers to customize their health programs towards the unique needs of itsa workforce.

Screening Services:
Over half of all serious diseases found in the U.S. are preventable. The screening tools offered by St. Luke's will help employers provide important health screening opportunities to employees such as:

St. Luke's Occupational Health On-Site Nurse Program provides qualified healthcare professionals to aid businesses in managing healthcare issues in the workplace. Through an assessment, monitoring, surveillance and clinical management, St. Luke's nursing staff will provide a comprehensive approach to meet the distinctive needs of each worksite.

Services provided through the On-Site Nurse program include:
