Rights & Responsibilities

St. Luke’s Health System is dedicated to providing health care services to patients in a courteous, respectful, and compassionate manner. We promise to provide you with:

  • Health care no matter what your race, color, creed, age, sex, national origin, disability, health status (including communicable disease) or source of payment.
  • Information in terms which you understand about your diagnosis, what will be done to help you, and what you may expect about your recovery.
  • Adequate information so you can participate in the plan for your treatment and consent to or refuse treatment. We will include Advanced Directives in your plan for treatment.
  • Privacy when you are treated, when your case is discussed, and when your medical records are reviewed.
  • Response to your request for services. When we are unable to provide a service, you may be transferred to another facility. But before you are transferred, you will receive complete information about the transfer and the other facility and the physician who has agreed to care for you.
  • Names of persons providing your care and treatment. Employees have name badges to help you.
  • Notice if your proposed treatment is experimental in nature. You may choose not to participate in such activity.
  • Information about health care services you will need or may wish to use after you leave the health care setting to help you recuperate.
  • Information about pain and pain relief measures.
  • Concerned staff members who will respond quickly to reports of pain.
  • A copy of your bill and an explanation of it, regardless of the source of payment, as law permits.

  • Help in resolving concerns about your care and access to protective services.

Your help is important as we care for you. Therefore, we ask that you do these things to help in your care:

  • Provide accurate and complete information about your present and past illnesses, treatments and medications, previous hospital admissions and other health matters, including your Advanced Directives.
  • Work with your caregivers to develop a pain management plan.
  • Cooperate with St. Luke’s employees as they care for you and treat you.
  • Be considerate of other patients by controlling your visitors, noise, and adhering to our smoking policy.
  • Respect St. Luke’s property.
  • Provide accurate financial and insurance information about payment of your bill.
  • Provide us with your opinion about the care you receive and your suggestions for improvement.
